1. Please ensure all information submitted are correct. An admin fee will be charged for reprinting of certificate.
2. An acknowledgement will be sent out to your email upon submission of this form.
3. The course dates will be sent to you once the course is confirmed.
4. In addition to the course fee, a S$35 registration fee is required.
5. Acceptable payment modes: Cash/NETS/Cheque.
6. Invoice can be issued for company sponsored participants (please indicate).
7. There will be no refund once the payment has been paid or if the class has commenced.
8. No make-up lessons provided if missed.
9. Computer terminals with the relevant software and lesson handouts will be provided.
